29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. (a) Yard 32 - On or about May 25, 2017, the employer did not ensure that specific energy control procedures were established to control hazardous energy during servicing and maintenance activities on the Bridgemaster trailer on the McNeilus, serial # 82755-31060, ready mix truck. The Lockout tag out procedure for the ready mix concrete trucks and mixers did not address locking out all energy sources such as the gravitational energy when performing maintenance on the Bridgemaster trailer in the elevated position. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Yard 32: On or about May 25, 2017, the employees were exposed to struck by hazards during servicing and maintenance activities on the Bridgemaster trailer on the McNeilus ready mix truck serial # 82755-31060. The employer failed to implement energy control application steps as lock out tag out devices were not applied to control gravitational energy when performing servicing and maintenance on the Bridgemaster trailer. (b) Yard 32 - On or about May 25, 2017, the employer did not ensure that each authorized employee affixed a personal locking device on the battery when working as a group to service the McNeilus Bridgemaster Ready Mix Truck serial #82755-31060. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification.
