29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch: An employee engaged in operating an 8 inch DeWALT bench grinder (Model DW758, SN 2013 26-YL92138) in the machine shop was exposed to struck-by hazards, in that the adjustable tongue was secured at approximately 1/2inch above the peripheral wheel.
