29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   On or about May 25, 2017, employees performing residential roofing installation operations at a residential property were exposed to a 12 foot fall hazard when the employer failed to ensure fall protection was utilized.    No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed:  a. On or about May 25, 2017, at the above referenced worksite, employees were required to gain access / egress to the  roof in the performance of roofing operations by use of a ladder that did not extend at least three (3) feet above the upper landing (roof edge). Employees were thereby exposed to a fall hazard of approximately twelve feet to the ground below.   There is no abatement certification of documentation required for this item.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   On or about May 25, 2017, employees performing residential roofing operations were exposed to an 12 foot fall when the employer failed to ensure that employees were adequately trained to recognize the hazards associated with working at elevation.      There is no abatement certification of documentation required for this item.
