29 CFR 1910.178(m)(8): There shall be sufficient headroom under overhead installations, lights, pipes, sprinkler system, etc.: a) On or about 5/25/2017 and at times prior thereto; the employer did not ensure a heating unit mounted directly in front of aisle P1 within the warehouse located at 10500 N. Stemmons freeway in Dallas, Texas had sufficient headroom and it was being contacted by the forklift and/or load as it is being placed approximately 19 feet up on the top shelf of the rack exposing employees to a burn hazard from a gas leak and fire. b) On or about 5/25/2017 and at times prior thereto; the employer did not ensure a natural gas pipe mounted directly over aisle P8 within the warehouse located at 10500 N. Stemmons freeway in Dallas, Texas had sufficient headroom and it was being contacted by the forklift and/or load as it is being placed approximately 19 feet up on the top shelf of the rack exposing employees to a burn hazard from a gas leak and fire. c) On or about 5/25/2017 and at times prior thereto; the employer did not ensure that three heating units mounted directly in front of aisle P8 within the warehouse located at 10500 N. Stemmons freeway in Dallas, Texas had sufficient headroom and it was being contacted by the forklift and/or load as it is being placed approximately 19 feet up on the top shelf of the rack exposing employees to a burn hazard from a gas leak and fire. d) On or about 5/25/2017 and at times prior thereto; the employer did not ensure a heating unit mounted directly over aisle P12 within the warehouse located at 10500 N. Stemmons freeway in Dallas, Texas had sufficient headroom and it was being contacted by the forklift and/or load as it is being placed approximately 19 feet up on the top shelf of the rack exposing employees to a burn hazard from a gas leak and fire. e) On or about 5/25/2017 and at times prior thereto; the employer did not ensure a heating unit mounted directly in front of aisle P16 within the warehouse located at 10500 N.
