29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. a. On or about May 25, 2017 at the above referenced worksite, employees were observed working in a hazardous area without a protective helmet, as required by this OSHA standard. As a result, employee was exposed to hazards associated with falling / flying objects. There is no abatement certification of documentation required for this item.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about May 25, 2017, at the above referenced worksite, employees engaged in residential construction activities on a residential roof were exposed to a fall hazard greater than 6 feet. The employer failed to ensure that all exposed employees utilized fall protection as required by this standard. Employees were thereby exposed to a fall hazard of approximately 10 feet. There is no abatement certification of documentation required for this item.

On or about May 25, 2017 at the above referenced worksite, employees engaged in residential construction were required to utilize a portable ladder for access to an upper landing surface. The employer failed to ensure the ladder was on a solid level surface and / or secured at its top to a rigid support that would not deflect. Employees were thereby exposed to a fall hazard of up to ten feet. There is no abatement certification of documentation required for this item.
