29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a. On or about August 5, 2020, at the above addressed jobsite,  employees using a pneumatic nail gun did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about August 5, 2020, at the above addressed jobsite, employees performing residential roofing operations, nineteen(19) feet four (4) inches above the ground were not protected from falls, thereby exposing the employees to fall hazards.  Big Brothers Construction Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 1235617, citation number 1 item number 2 and was affirmed as a final order September 5, 2017 with respect to a workplace located at 317 23rd Avenue, Ottawa, IL 61350.

On or about August 5, 2020, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
