29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about May 22, 2017 and at times prior thereto, the employer did not ensure employees of a subcontractor, performing roofing work activities from a roof with unguarded skylight(s) were in use of fall protection. This exposed those employees to a fall hazard greater than 6 feet to the concrete/equipment below.

On or about May 22, 2017 and at times prior thereto, the employer did not ensure employees of a subcontractor, performing roofing work activities from a roof with open sides and edges were in use of fall protection. This exposed those employees to a fall greater than six(6) feet to the concrete/equipment below.
