29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    On or about May 23, 2017, the employer failed to ensure that employees of the subcontractor engaged in roofing activities including tear off and shingling were protected by flying particles. Employees were utilizing pneumatic air guns without out the use of safety rated glasses.

On or about May 23, 2017, the employer failed to ensure that subcontractor employees working from a ladder jack scaffold were protected from falling to the level below by the use of personal fall arrest systems.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       On or about May 23, 2017, the employer failed to ensure that subcontractor employees engaged in roofing activities of a residential home were protected from falling to the ground below by use of fall protection devices. Employees were working at height varying between 15 and 25 feet on a roof having a 10/12 pitch without the use of fall protection devices.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about May 23, 2017, the employer failed to ensure that ladders used by subcontractor employees for ascending and descending from a residential roof were extended at least 3 feet. Employees were utilizing the ladders to access the roof to perform shingle work.
