29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a. On May 30, 2017, on the 6th floor flat roof, employees performing roofing work were not protected from a 54 feet fall hazard by the use of a fall protection system.     a. On May 30, 2017, on the 7th floor flat roof, employees performing roofing work were not protected from a 63 feet fall hazard by the use of a fall protection system.     a. On May 30, 2017, on the 7th floor flat roof, employees performing roofing work were not protected from a 9 feet fall hazard by the use of a fall protection system.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:     a. On May 30, 2017, the two ladders used to access the 6th floor roof did not extend 3 feet above the landing, exposing employees to a 9 feet fall hazard.

29 CFR 1926.1053(b)(8): Ladders placed in any location where they can be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, or a barricade shall be used to keep the activities or traffic away from the ladder:    a. On May 30, 2017, the two ladders used to access the 6th floor roof, ocated in the stairwell, were not secured from accidental displacement.
