29 CFR 1910.269(c)(4)(ii)(B): The employee could not be expected to recognize and avoid the hazards involved in the job and a more extensive briefing was not held; a) On or about 05/18/17 at the PerOxychem substation located at 78 Sawyer Avenue, Tonawanda, New York; employees were performing preventative maintenance on the breakers in the substation. While preparing to work on breaker #6, an employee contacted live equipment and suffered burns and fractures. The breaker # 6 was energized differently than the 3 previous breakers that were serviced the day before. A secondary or redundant source of energy was present and was not discussed with the specific differences or procedures prior to work commencing. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.269(m)(3)(ii): The employer did not ensure that all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be denenergized were open. The employer did not render such means inoperable, unless its design does not so permit, and then sure that such means are tagged to indicate that employees are at work: a) On or about 05/18/17 at the PerOxychem substation located at 78 Sawyer Avenue, Tonawanda, New York; employees were performing preventative maintenance on the breakers in the substation. While preparing to work on breaker #6, an employee contacted live equipment and suffered burns and fractures. Switch #10 was left closed causing a backfeed from breaker #2 to breaker #6. NO ABATEMENT CERTIFICATION REQUIRED

While preparing to work on breaker #6, an employee contacted live equipment and suffered burns and fractures. The equipment was not tested prior to beginning work to ensure it was deenergized. NO ABATEMENT CERTIFICATION REQUIRED
