29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements:      (a) On or about May 24th, 2017, and at certain times before, employees accessing the "mezzanine" level within the establishment to stock and retrieve restaurant supplies were exposed to trip and fall hazards in that the stairway they were using lacked a handrail as required by standard.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:      (a) On or about May 24th, 2017, and at certain times before, employees were exposed to electrical shock, electrocution and fire hazards in that access to a breaker panel and one service entrance panel lacked the 3-foot frontal clearance required by standard.

29 CFR 1910.305(c)(4): Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface:      (a) On or about May 24th, 2017, and at certain times before, employees were exposed to electrical shock and burn hazards in that a light switch just inside one of the stairway lower landings had a broken faceplate resulting in exposed energized parts.

29 CFR 1910.1000(a)(2): Employees were exposed to an airborne concentration of carbon monoxide (CO), listed in Table Z-1, in excess of the 8-hour Time Weighted Average (TWA) concentration of 50 Parts Per Million (PPM):    (a) On or about May 24, 2017: An employee working in the kitchen was exposed to 208 parts carbon monoxide per million parts of air as determined by the employee's Carboxyhemoglobin (%COHb) medical blood gas measurement following the employee's exposures to carbon monoxide (CO).     (b) On or about May 24, 2017: An employee working in the kitchen and other areas of the facility was exposed to 89 parts carbon monoxide per million parts of air as determined by the employee's Carboxyhemoglobin (%COHb) medical blood gas measurement following the employee's exposures to carbon monoxide (CO).     Pursuant to 29 CFR 1903.19, within ten (10) days of the abatement date, the employer must submit documents showing that it is in compliance with the standard, including the steps it is taking to assure each employee is protected from carbon monoxide exposure 50 PPM or greater.    Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

(1) On or about May 24, 2017: An employee working in the kitchen was exposed to 208 parts carbon monoxide per million parts of air as determined by the employee's Carboxyhemoglobin (%COHb) medical blood gas measurement following the employee's exposures to carbon monoxide (CO).
