29 CFR 1910.29(b)(1): (1) The top edge height of top rails, or equivalent guardrail system members, are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface.  The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section (see Figure D-11).     a. On or about May 24, 2017, located at the flat pouring line auger, the elevated platform where employees stood to spray out the auger had a top rail approximately 32 inches above the walking-working surface, thereby exposing employees to a fall hazard of approximately six feet.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.     a. On or about May 24, 2017, located at the end of the flat pouring line, before the takeoff rollers on the East side, near the stackers workstation, there was an unguarded ingoing nip point created by the end conveyor pulley, thereby exposing employees to a caught-in hazard.       b. On or about May 24, 2017, located at the end of the flat pouring line, before the takeoff rollers on the West side, near the stackers workstation, there was an unguarded ingoing nip point created by the end conveyor pulley, thereby exposing employees to a caught-in hazard.       c. On or about May 24, 2017, located at the end of the flat pouring line takeoff rollers, near the stackers workstation, there was an unguarded ingoing nip point created by the end conveyor pulley, thereby exposing employees to a caught-in hazard.       d. On or about May 24, 2017, located in the warehouse area, an employee operated a Wulftec stretch wrap machine, model WHP-150, that did not have guarding to prevent contact with the rotating parts, thereby exposing employees to a caught-in/struck-by hazard.       e. On or about May 24, 2017, located in the warehouse area, an employee operated a Wulftec stretch wrap machine, model WHP-200, that did not have guarding to prevent contact with the rotating parts, thereby exposing employees to a caught-in/struck-by hazard.

29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves.     a. On or about May 24, 2017, located at the front of the flat pouring line, where employees loaded molds onto the powered rollers, there were multiple unguarded one inch diameter shaft ends that projected one inch or more, thereby exposing employees to a caught-in hazard.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.    a. On or about May 24, 2017, located on the East side of the flat pouring line at the tint table, a relocatable power tap was used as a substitute for fixed wiring for supplying power to a Milwaukee hand-held drill used to mix paint and a wall-mounted fan, therefore not used in accordance with the UL listing and exposing employees to a fire hazard.
