29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye protection when exposed to eye or face hazards from flying particles: On or about 31 May 2017, an employee was using a pneumatic nail gun to conduct framing activities on the roof of a residential home under construction up to approximately 18.5 feet high without the use of eye protection, exposing the employee to the hazard of being struck by flying particles.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8m) or more above the lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems. On or about 31 May 2017, three employees were conducting framing activities on the roof of a residential home under construction at heights up to 18.5 feet without the use of fall protection, exposing the employees to a fall hazard.

29 CFR 1926.1053(a)(24): The side rails of the ladder did not extend 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder. On or about 31 May 2017, employees were using a job-made wooden ladder built into the side framing of a residential home under construction that did not have side rails extending 42 inches above the upper landing that they were accessing exposing the employees to a fall hazard.
