29 CFR 1926.403(b)(1):     Examination, The employer did not ensure that electrical equipment was free from recognized hazards that are likely to cause death or serious physical harm to employees.     On 5/9/2017, employees from Trinity Masonry and Tuckpointing LLC were performing masonry work on a commercial building located at 3715 Natural Bridge Road, Saint Louis MO.   Employer did not ensure the Sawmaster SDT-1410 Masonry saw was free from recognized hazards; the cord had damaged insulation with exposed conductors.  The employees were using the masonry saw to cut brick.

29 CFR 1910.134(c)(1):   A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i)  (ix) with worksite specific procedures was not established and implemented for required respirator use.    On 5/9/2017, employees from Trinity Masonry and Tuckpointing LLC were performing masonry work on a commercial building located at 3715 Natural Bridge Road, Saint Louis MO.   Employees were using a Sawmaster SDT-1410 masonry saw to cut brick, and using a Gerson 8300 series half face cartridge respirator to protect themselves from breathing silica dust.  The employer did not establish a written respiratory protection program with worksite specific procedures.

Employees were using a Sawmaster SDT-1410 masonry saw to cut brick, and using a Gerson 8300 series cartridge half face respirator to protect themselves from breathing silica dust.  The employer did not provide a medical evaluation to determine the employee(s) ability to wear a respirator before use in the workplace.

The employer did not ensure that an appropriate qualitative fit test or quantitative fit test was performed before initial use and performed annually thereafter.
