29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift:     a.  On or about May 31, 2017 at the above addressed jobsite, an employee working in an aerial lift was not secured from falling out of the basket by personal fall protection equipment attached to the boom or basket by the use of a lanyard. The employee was exposed to a fall of more than fifteen (15) feet.     There is no abatement certification or documentation required for this item.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:   a.  On or about May 31, 2017 at the above addressed jobsite, an employee using an aerial lift to perform work on the outside of the building was not authorized and or trained to recognize the hazards associated with the use of aerial lifts, thereby exposing the employee to the hazards associated with falls.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
