29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were the employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):      a)  On June 1, 2017, employees were exposed to fall hazards of up to approximately 20.4 feet while performing roofing work to an existing single family home.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:    a)  On June 1, 2017, employees were using three extension ladders which did not extend 3 feet above the upper landing being accessed.
