29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about June 1, 2017, at the above addressed jobsite, an employee was exposed to flying particles when using a pneumatic nail gun without face or eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 1, 2017, at the above addressed jobsite, employees engaged in residential construction were exposed to a 20-foot fall hazard without means of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 1, 2017, at the above addressed jobsite, employees were exposed to a 10-foot fall hazard when the ladder used to access the second level of the residence was not extended 3 feet above the upper landing surface.
