29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    (a)  On or about June 1, 2017, the employer did not develop and document a Hazardous Energy Control Program to include energy control procedures when employees were performing servicing and maintenance activities.    (b)  On or about June 1, 2017, the employer did not develop and document a Hazardous Energy Control Program to include employee training when employees were performing servicing and maintenance activities.    (c)  On or about June 1, 2017, the employer did not develop and document a Hazardous Energy Control Program to include periodic inspections when employees were performing servicing and maintenance activities.

29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves.    (a)  On or about June 1, 2017 in the Post Peeler area, on the southwest (east side) end of the infeed roller adjacent to the post peeler had three 1 3/4" diameter shafts which projected approximately 2 3/4" exposing employees to caught in hazards.    (b)  On or about June 1, 2017 in the Post Peeler area, the bark conveyor leading to the inclined drag conveyor had an unsmooth 1" diameter shaft which projected approximately 2 1/2" exposing employees to caught in hazards.    (c)  On or about June 1, 2017 in the Post Peeler area, the incline drag conveyor on the southwest side had a 1" diameter shaft which projected approximately 3 1/2" exposing employees to caught in hazards.    (d)  On or about June 1, 2017 in the Post Peeler area, the northeast side of the inclined drag conveyor had a shaft which projected more than one-half the diameter exposing employees to caught in hazards.    (e)  On or about June 1, 2017 in the Post Peeler area, the south side of the first outfeed roller from the post peeler had five 1 3/4" diameter projecting keyway shafts which projected between 1" to 2 7/8" exposing employees to caught in hazards.    (f)  On or about June 1, 2017 in the Post Peeler area, the southwest corner of the first outfeed belt conveyor had a 1 1/2" diameter shaft which projected approximately 1 3/4" exposing employees to caught in hazards.    (g)  On or about June 1, 2017 in the Post Peeler area, the northeast corner of the first outfeed belt conveyor had a 1 1/4" diameter shaft which projected approximately 1 1/2" exposing employees to caught in hazards.    (h)  On or about June 1, 2017 in the Post Peeler area, the northwest corner of the first outfeed belt conveyor had a 1 1/4" diameter shaft which projected approximately 1" exposing employees to caught in hazards.

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.    (a)  On or about June 1, 2017 in the Post Peeler area, on the northwest side of the inclined drag conveyor had a belt and pulley assembly was not guarded exposing employees to caught in hazards. 29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.    (a)  On or about June 1, 2017 in the Post Peeler area, the Morbark (Model:  PSP, S/N:  251) had unguarded chain and sprockets which were unguarded exposing employees to caught-in hazards.  These areas included, but are not limited to:    1.  southeast corner of the infeed conveyor,    2.  six exposed on the southwest end of the infeed roller,    3.  five exposed on the south east side of the second infeed roller (opposite the log reject station)    4.  southeast end of the infeed roller adjacent to the post peeler    5.  southwest corner of the post peeler,    6.  northeast side of the inclined drag conveyor,    7.  five exposed on the south end of the first outfeed roller,    8.  southeast end of the first outfeed belt conveyor,     9.  northeast end of the second outfeed belt conveyor.    10.  northeast corner of the post peeler,    11.  north east corner of the bark conveyor.    12. four under the rubber mat located to the east of the log reject station.

29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the considerations outlined in 29 CFR 1910.303(b)(1)(i-viii):    (a)  On or about June 1, 2017, in the Post Peeler area the flexible conduit, schedule 40 electrical conduit, and electrical junction boxes were damaged and broken by moving equipment in this area. The damage areas had  exposed electrical wiring exposing employees potential  electrical shock, burns, fire, and arc flash hazards.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means:  (a)  On or about June 1, 2017 at the Post Peeler Area, three oval junction boxes coming from the T.P.C. Drives, Speedpak electrical disconnect were damaged exposing employees to electrical shock, burns, and fire.  (b)  On or about June 1, 2017 at the Post Peeler Area, the flexible electrical conduit between the infeed rollers used to power the solenoid was damaged exposing employees to electrical shock, burns, and fire.  (c)  On or about June 1, 2017 at the Post Peeler Area, two 40 Schedule conduit pipes leading to the Motion Industries, Inc. hydraulic system was damaged in multiple location exposing employees to electrical shock, burns, and fire.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed.  (a)  On or about June 1, 2017 at the Post Peeler Area, the junction box located on the infeed conveyor was not closed in that the bottom of the cover was open with wire nuts visible.
