29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about 6/1/2017, during residential roofing activities, located at 7468 Hawksfield Rd Canal Fulton OH 44614, the employer failed to ensure that the ladders used to access the roof were at least three feet above the landing surface. The ladders in use to gain access to the roof were about 1.5 feet above the landing surface.
