29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a) At a commercial construction site located at 222 Main St, Tarkio, MO: The employer failed to protect employees from possible head injuries from impact, or from falling or flying objects. This was demonstrated on 5/25/2017 when the employer allowed two employees to work inside an active demolition site that included compromised ceilings overhead and the use of an excavator to remove materials where employees were within 10 feet.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) At a commercial construction site located at 222 Main St, Tarkio, MO the employer failed to protect employees from the exposure of flying particles. This was demonstrated on 5/25/2017 when two employees were permitted to work inside the demolition site without wearing eye protection while an excavator was being used to remove materials. Employees were located within 10 feet of the bucket while it was in use.

29 CFR 1926.859(g): During demolition, continuing inspections by a competent person were not made as the work progressed to detect hazards resulting from weakened or deteriorated floors, or walls, or loosened material. Employees were also permitted to work where such hazards existed without being corrected by shoring, bracing, or other effective means. a) At a commercial construction site located at 222 Main St, Tarkio, MO: Employees were exposed to safety and health hazards during the work process that were not identified through continuous inspections of weakened or deteriorated floors, walls and loosed material. This was demonstrated on 5/25/2017 when employees were permitted to stand on the second story of the building while an excavator was being used to remove demolished materials.
