29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  a) On May 31, 2017, employees were exposed to fall hazards while working on a red tubular welded frame scaffold that was not fully planked.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  a) On May 31, 2017, employees were exposed to fall hazards while using a red tubular welded frame scaffold that did not have base plates.  The scaffold legs rested on plywood on the sidewalk and over a drainage grate.  b) On May 31, 2017, employees were exposed to fall hazards while working on a yellow tubular welded frame scaffold without base plates.

29 CFR 1926.451(c)(1)(ii): Supported scaffolds with a height to base width ratio of more than four to one (4:1) were not restrained from tipping by installing guys, ties, and braces:  a) On May 31, 2017, a red tubular welded frame scaffold in excess of the 4:1 height to base ratio was not secured to prevent tipping, exposing employees to fall hazards.  b) On May 31, 2017, a yellow tubular welded frame scaffold in excess of the 4:1 height to base ratio was not secured to prevent tipping, exposing employees to fall hazards.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) On May 31, 2017, employees were exposed to a fall of approximately eleven feet to the concrete sidewalk below while working from a yellow tubular welded frame scaffold without guardrails.    b) On May 31, 2017, employees were exposed to a fall of up to 22 feet to the concrete sidewalk below while working from a red tubular welded frame scaffold without guardrails.
