29 CFR 1926.753(c)(2): A qualified rigger did not inspect the rigging prior to each shift in accordance with 29 CFR 1926.251: On or about May 23, 2017, 4099 Embassy Parkway, Fairlawn, OH, 44333, during steel erection activities including but not limited to hoisting and landing metal decking bundles on a structural steel building, employees were exposed to struck-by hazards when the employer did not ensure all rigging was inspected by a qualified rigger prior to use and taken out of service when defective. A wire rope sling with several kinks was in use.

29 CFR 1926.757(e)(4): No bundle of decking may be placed on steel joists until all bridging has been installed and anchored and all joist bearing ends attached, unless all of the following conditions are met: On or about May 23, 2017, 4099 Embassy Parkway, Fairlawn, OH, 44333, during steel erection activities including but not limited to hoisting and landing metal decking bundles on a structural steel building, employees were exposed to fall and collapse hazards approximately 15.7 to 18.9 feet when the employer did not ensure at least one row of bridging was installed and anchored or a qualified person determined and documented the structure was capable of supporting the load.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a. On or about May 23, 2017, at a worksite located at 4099 Embassy Parkway, Fairlawn, OH, 44333; during structural steel erection activities including but not limited to detail work, the employer did not ensure employees were protected from fall hazards ranging from 15.7 to 18.9 feet by an adequate fall protection system/device. Employees were observed performing final detail work activities with no fall protection. b. On or about May 23, 2017, at a worksite located at 4099 Embassy Parkway, Fairlawn, OH, 44333; during structural steel erection activities including but not limited to detail work, the employer did not ensure employees were protected from fall hazards ranging from 15.7 to 18.9 feet by an adequate fall protection system/device. Employees were observed performing hoisting and landing metal decking activities with no fall protection. c. On or about May 23, 2017, at a worksite located at 4099 Embassy Parkway, Fairlawn, OH, 44333; during structural steel erection activities including but not limited to detail work, the employer did not ensure an employee was protected from fall hazards ranging from 15.7 to 18.9 feet by an adequate fall protection system/device. The employee was observed performing supervisory activities on the structural steel with no fall protection.
