29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: The employer is failing to protect employees from fall hazards when conducting roofing operations on a commercial structure. This was most recently documented at 2545 Cornhusker Highway, Lincoln, NE on or about 1 June 2017 when employees were observed removing old roofing materials and installing composite shingles to a commercial hotel structure while working greater than 6 feet above the ground without fall protection use.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: The employer is failing to protect employees from fall hazards when conducting roofing operations on a commercial structure. This was most recently documented at 2524 Cornhusker Hwy, Lincoln, NE, on or about 1 June 2017 when 5 employees were observed using a ladder to access a roof that was approximately 30' high and the ladder's side rails did not extend at least 3' past the upper landing surface of the roof.
