29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: On and about May 31st, 2017, employees engaged in gutter cleaning activities on the residential roof were exposed to fall hazards of approximately 8 feet from the eave of the residence to the ground below in that fall protection was not utilized.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Employees engaged in tree trimming activities were exposed to struck by hazards from fall objects in that a hard hat was not utilized when working below overhead work. (b) Employees engaged in limb cutting activities using chain saws were exposed to struck-by hazards in that chaps were not utilized. (c) Employees engaged in landscaping activities using hazardous chemicals including but not limited to Coron 28-0-0, Trimec 992 broadleaf herbicide and Dimension 2EW herbicide, were exposed to health hazards in that goggles were not utilized.
