29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe-boards: Note:  Toe-boards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.      In the Production Area, the overhead storage area that stored boxes of supplies, was not equipped with a guard rail, mid-rail, and toeboards, exposing employees to an unprotected fall hazard of approximately 8 feet.

29 CFR 1910.133(a)(3): 29 CFR 1910.133(a)(3):  The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.    In the production shop, employees who wore prescription lenses that were not neither ANSI approved safety glasses nor designed to provide eye protection while operating the Punch press and radial arm saws, exposing employees to the hazard of eye injuries.
