This was most recently documented at 2406 J St, Lincoln, NE, on or about June 2, 2017 when employees were observed on a 2 story commercial building and they climbed a metal ladder that extended under an energized power line.

This was most recently documented at 2406 J St, Lincoln, NE, on or about 6/2/2017 when employees were observed installing shingles to a commercial structure while working greater than 6 feet above the ground without fall protection in use.

This was most recently observed at 2406 J St, Lincoln NE, on or about June 2, 2017 when employees were using a non self supporting ladder that was not positioned at a safe working angle.  The horizontal distance of the top support to the foot of the ladder was greater than one quarter of the working length of the ladder.
