29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    The employer is failing to protect employees from electrical hazards when conducting roofing operations on a commercial structure.  This was most recently documented at 2406 J St, Lincoln, NE, on or about June 2, 2017 when employees were observed on a 2 story commercial building and they climbed a metal ladder that extended under an energized power line.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    The employer is failing to protect employees from fall hazards when conducting roofing operations on a commercial structure with a steep roof.  This was most recently documented at 2406 J St, Lincoln, NE, on or about 6/2/2017 when employees were observed installing shingles to a commercial structure while working greater than 6 feet above the ground without fall protection in use.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:    The employer is failing to protect employees from fall hazards when conducting roofing operations on a commercial structure.  This was most recently observed at 2406 J St, Lincoln NE, on or about June 2, 2017 when employees were using a non self supporting ladder that was not positioned at a safe working angle.  The horizontal distance of the top support to the foot of the ladder was greater than one quarter of the working length of the ladder.
