29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about 5/16/2017 at buildings 300 at 315 Oconee Street, Athens, GA: Employees using and working on the mast climbing scaffold systems at heights up to or above 40 feet from ground level used the mast tower metal frame without fall protection to access and egress the scaffold system(s).

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  On or about 5/16/2017 at buildings 300 at 315 Oconee Street, Athens, GA: Employees using and working on the mast climbing scaffold system at heights up to or above 40 feet were exposed to fall hazards from the open sides and ends of the scaffolding systems with missing guardrails or fall protection.

29 CFR 1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.  On or about 5/16/2017 at buildings 300 at 315 Oconee Street, Athens, GA: Employer failed to ensure all employees erecting, dismantling, operating, inspecting, or moving the mast climbing scaffolding systems at heights at or above 40 feet from ground level were trained by a competent person to recognize hazards such as but not limited to installation and securing of outrigger planks, guardrail installation, cribbing and securing support jacks.
