29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, was not worn and attached to the boom or basket when working from an aerial lift: Northeast side of UTSA Blvd: On June 5, 2017 and at times prior thereto, an employee exposed to a fall hazard over 25 feet to the ground below while installing fiber optic line from a vehicle mounted aerial lift man basket, was not utilizing a personal fall arrestsystem.

29 CFR 1910.67(c)(2)(viii): An aerial lift truck was moved when the boom was elevated in a working position with men in the basket, except for equipment which is specifically designed for this type of operation in accordance with the provisions of paragraphs (b)(1) and (b)(2) of this section: Northeast side of UTSA Blvd: On or about June 5, 2017, an employee was exposed to a fall hazard, when an aerial lift truck, was moved when the boom was elevated in a working position with a worker in the basket.
