29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Maintenance employee(s) of Saker ShopRites, Inc. supported construction / demolition activities in the new addition at the store at 2909 Washington Rd in Parlin NJ. Duties included operation of forklift truck(s) to change out dumpsters full of debris from concrete block wall demolition. Operator(s) of forklifts had not been trained and evaluated for competency in safe truck operation; exposing employee(s) to struck-by hazards from falling loads and/or vehicle(s). Condition(s) were noted on or about 6/01/2017.
