On or about June 7, 2017, Warehouse: The Toyota Model 8FGCU25 fork truck was missing the manufacturer's installed load backrest extension. The modification was made without the manufacturer's prior written approval.

29 CFR 1910.303(g)(1)(iii): The employer did not ensure that at least one entrance of sufficient area was provided to give access to the working space about electric equipment 600 Volts, nominal, or less: A. On or about June 7 ,2017, Warehouse: Access to the GE A-Series 120/208V electrical breaker panel was blocked by the storage of palletized spools of wire. The working space in front of the panel was clear, but no access to the working space was maintained.
