29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: a) Jobsite - Employees were working from extension ladders that were placed inside of a dumpster with debris to assist in the installation of windows on or about 5/24/2017. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: a) Jobsite - Employees using extension ladders to assist in the installation of windows had not been provided ladder safety training on or about 5/24/2017. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
