29 CFR 1915.74(a)(4): The gangway was not kept properly trimmed at all times. Pier 3, Halo, Port Side: Employer permitted employees to board or leave the vessel by using a gangway that was not kept properly trimmed at all times. The gangway was positioned approximately twenty four (24) inches away from the vessel and twenty four six (50) inches above the deck.
