On or about June 6, 2017, at the construction site located at 286 Ensign Lane, Lewis Center, Ohio, employees were performing roofing work on a new two story residential home with a pitch of approximately 6 in 12 and approximately 20 feet in height without fall protection, thereby exposing the employees to a fall hazard.
