29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Maintenance/Carpenter Shops: The electric drills did not have guards at the chuck exposing operators and workers to rotating hazards when used to drill metal, plastic and wood for building maintenance. (1) Dayton- Model # 5Z1468 was not guarded. (2) Delta- Model # 17965 was not guarded.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: In the following instances, sufficient working space was not available: A. Warehouse - B&U: The electric panel load center at LW3 controlling the lights and outlets was blocked by a Clark forklift -S/N 0800182-19 - Model 120. The forklift was parked and out of service. B. Warehouse- Pole Barn: The electric panel load center and Transformer- TDP1 controlling the lights and outlets for transformer (TW1) was blocked by stored materials including doors, sheetrock, mower and drum at /LW1/3/5/preventing direct access for operators and maintenance employees.
