29 CFR 1926.1408(a)(2): The equipment's maximum working radius could get closer than 20 feet to a power line and the employer did not meet the requirements in Option (1), Option (2), or Option (3) of this section:   On May 22, 2017 at the south side of the Imperial Pacific Resort Hotel project, the tower crane load line used to lift steel rebar touched a power line, exposing employees to electrocution hazards.

29 CFR 1926.1428(a): The employer of the signal person must ensure that each signal person meets the Qualification Requirements (paragraph (c) of this section) prior to giving any signals:   On May 22, 2017 at the south side of the Imperial Pacific Resort Hotel project, the employee assigned to signal the crane operator while lifting metal rebar was not trained and qualified, exposing employees to struck-by and electrocution hazards.
