29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a) 650 Westerly Pkwy State College PA, - Employees were observed performing roofing activity on a low-slope school roof, without the benefit of a fall protection system, thereby exposing employees to a fall of approximately 44 feet to the ground below, on or about June 1, 2017. No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) 650 Westerly Pkwy State College, PA - Employees were using a fiberglass extension ladder that did not extend at least 3 feet above the landing surface to the buildings hatch, thereby exposing employees to a fall hazard of approximately 16 feet to the concrete floor below, on or about June 1, 2017. No abatement certification or documentation required.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) 650 Westerly Pkwy State College, PA - The employer did not provide training to employee(s) which would have allowed employee(s) to recognize the hazards of falling and the equipment and procedures to use to minimize those hazards, thereby exposing employees to possible injuries, on or about June 1, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
