29 CFR 1910.305(j)(4)(v): The disconnecting means was not readily accessible:  If more than one disconnect is provided for the same equipment, only one need be readily accessible.    (a) Yard- June 6, 2016, the employer did not ensure that a means to disconnect was readily accessible for the pallet disassembler. The control button did not work and was disconnected; employees had to access an electrical panel 15 feet away to power the machine by switching the circuit breaker to the on/off position.
