29 CFR 1910.28(b)(3)(iv) as require by 1960.8(b): The employer did not ensure each employee was protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used.  On or about June 5, 2017, in the boiler room, a floor opening access leading down to the elevator control room was not guarded with a guardrail system, there by exposing employees to a fall hazard.

29 CFR 1910.303(g)(1)(ii) as required by 1960.8 (b): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:   On or about June 6, 2017, in the TELCO room, employees were exposed to electrical hazards in that work space in front of electrical panels was used for storage of large box containing a Cathode ray tube (CRT).  Employer did not provide and maintain sufficient access which could hinder the ready and safe operation and maintenance of equipment.

29 CFR 1910.305(a)(2)(ix) as require by 1960.8(b): All lamps for general illumination shall be protected from accidental contact or breakage by a suitable fixture or lampholder with a guard. Brass shell, paper-lined sockets, or other metal-cased sockets may not be used unless the shell is grounded.   On or about June 6, 2017,  in the tower stairwell near the fourth floor, a 26 watt CFL which contained mercury was used to illuminate the stairwell was not guarded to prevent accidental contact or breakage there by exposing employees to an electrical shock and mercury hazards.

29 CFR 1910.1200(g)(1) as required by 1960.8(b): The employer did not have a safety data sheet (SDS) for each hazardous chemical in use:   On or about June 6, 2017, in the mechanical room, employees were required to use Lubricating Oil and SDSs for the chemical was not available.  Employees were exposed to ingestion, eye, and skin contact hazards that could result in health effects such as, but not limited to irritation of  mucous membranes eyes, and skin.
