29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) For the shop/storage location, as observed on or about 06/06/2017, a sit-down forklift was operated without a seat belt. This conditions exposed to employees to potential struck-by hazards in event of a forklift tip-over.
