29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    The employer is failing to ensure that all employees exposed to eye and face hazards from flying particles while working on a commercial roof.  This most recently occurred on June 6, 2017 on the jobsite located at 2505 Farefield St, Lincoln, NE 68521.  Employees were observed using pneumatic nail guns to install roofing material without any form of eye or face protection.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    The employer is failing to ensure that all employees exposed to fall hazards while working on a steep-sloped commercial roof are utilizing fall protection.  This most recently occurred on June 6, 2017 on the jobsite located at 2505 Farefield St, Lincoln, NE 68521.  Employees were observed re-roofing a 3-story hotel with a 5 in 12 and 10 in 12 sloped roof at heights greater than 6 feet above the ground.

This most recently occurred on June 6, 2017 on the jobsite located at 2505 Farefield St, Lincoln, NE 68521.  Employees are observed using the ladders which were divided by a landing surface to gain access to a 3-story roof where the ladders were not clear of obstructions.

29 CFR 1926.1053(a)(8): A metal spreader or locking device shall be provided on each stepladder to hold the front and back sections in an open position when the ladder is being used:    The employer is failing to ensure that all employees are protected from fall hazards when using a portable A-framed stepladder to gain access and egress a commercial roof.  This most recently occurred on June 6, 2017 on the jobsite located at 2505 Farefield St, Lincoln, NE 68521.  The employer had employees using a portable A-framed stepladder to gain access to the roof of a commercial building for re-roofing activities.  The stepladder is being used to transition between two aluminum extension ladders.  The A-frame ladder is in the closed position and leaning against the building instead of being in the open position with the metal spreader locked in place.

This most recently occurred on June 6, 2017 on the jobsite located at 2505 Farefield St, Lincoln, NE 68521.  Employees are observed using two ladders divided by a landing surface to gain access to a 3-story roof where the bottom ladder is supported by the top rung instead of the two rails.
