The employees were working from a supported scaffold while installing masonry materials on a newly constructed residential home located in Avon Lake, Ohio 44012. The employees on the ground were working under employees on the scaffold without wearing protective helmets. Julio Cortes was previously cited for a violation of this occupational safety and health standard 1926.100(a), which was contained in OSHA inspection number 1152864, citation number 1, item number 1 and was affirmed as a final order on 07/26/2016, with respect to a workplace located at lot 44 Portsouth, Brunswick, Ohio 44212.

The upper level of the scaffold was not fully planked. The second level of the scaffold was not fully planked.
