29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: moving parts of the machine.  On or about June 1, 2017, an employee was exposed to the hazards created by the moving ejector plate, on the Kawagguchi Injection Molding Machine, Press # 8, as it retracted back into the frame of the mold after the cycle ended.  The ejector plate pulled the employee's hand into the machine while she was attempting to remove a part.  Machine guarding was not provided to protect the operator from the movement of the ejector plate.  This item does not require certification or documentation.
