29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about June 2, 2017 and at times prior, the employer, D & B Precision Products, Inc., did not ensure employees, who were required to operate a JET horizontal band saw, were protected from amputation hazards. b) On or about June 2, 2017 and at times prior, the employer, D & B Precision Products, Inc., did not ensure employees, who were required to operate an Amada AC Servo press brake, were protected from crushing hazards.

29 CFR 1910.1026(d)(1): The employer with a workplace or work operation covered by this standard did not determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI): a) On or about June 2, 2017 and at times prior, the employer, D & B Precision Products, Inc., did not ensure employees, who were required to engage in stainless steel welding operations, were protected from hazards associated with exposure to hexavalent chromium. The employer had not determined the 8-hour time-weighted average exposure for employees exposed to hexavalent chromium.
