29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel:  On or about June 2, 2017, employees were exposed to struck-by hazards when the employer had not determined nor trained the employees in the driving procedures that they were to use to insure that the drivers had a clear view of the path of travel when driving powered industrial trucks out of or into rack aisles.  The employer has product that is 12 or more feet long.  Some of the powered industrial trucks have forks that are 8-12 feet long.  Due to the length of the forks, several feet of the product/forks would be into the shipping/receiving lane or aisle before the driver could see around the rack to see if a pedestrian or powered industrial truck was approaching.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  On or about June 2, 2017, employees were exposed to fire hazards when  a. there were no fire extinguisher signs on the fire extinguishers that were mounted on the walls and the racks. b. a fire extinguisher near the exit door was blocked by materials in front of the exit. The tests that were provided did not include the evaluation date or the name of the trainer. The tests were considered the certification for employees authorized to drive powered industrial trucks.
