29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about June 07, 2017, and at times prior thereto, the job made Noodler machine was not equipped with machine guards to prevent employees' hands from being caught in the machine.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling.  On or about June 07, 2017, and at times prior thereto, various electrical power tool handles had been disassembled and were used as part of job built Noodler Machines, four were found being used in the production area, exposing employees to the hazards of electrical shock.

29 CFR 1904.29(b)(4): 29 CFR 1904.29(b)(4): A form with equivalent information to OSHAs 301, Injury and Illness Incident Report, was not used by the employer to document each recordable injury and illness.  At the inspection site, for year 2017, the employer used a worker's compensation form for recordable injuries.  The forms were missing such information as instructions regarding what the injured worker was doing/equipment used just before the injury, and some forms were not completely filled out, lacking such information as the preparer's identity, date lost time began, and the like.
