29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: At the commercial construction site located at 3950 N Mulberry Street Kansas City, Missouri: Employees were exposed to an electrical shock hazard. Employees engaged in commercial framing activities were exposed to the 110 volt energized conductors on the repaired extension cord. Conductors were exposed on the cord providing power to a skil saw.

29 CFR 1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, or similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers: At the commercial construction site located at 3950 N Mulberry Street Kansas City, Missouri: Employees engaged in commercial construction activities were exposed to a fall hazard when a guardrail system was not in place around a completed excavation. There was one open side of the excavation and mid-rails and top-rails missing from the other sides resulting in a fall hazard of approximately 10 feet from the top-rail and 8 feet from the mid-rail and 6 feet 6 inches from deck of excavation.
