29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  On or about 7 June 2017, and at times prior thereto, various hand-held dispensing bottles within the shop floor contained flammable liquids that were not labeled to the contents or hazard of the chemicals, exposing the employees using them to a fire hazard.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation.  On or about 7 June 2017, the employer did not complete the OSHA 300 Log of Work-Related Injuries and Illnesses in the detail as required by the regulation. 1) For calendar years 2014, 2016 and 2017, Column F, which requires a description of the injury or illness, parts of body affected, and the object/substance that directly injured or made the person ill, was missing the specific injury for each case that was recorded. 2) Calendar year 2014 had one case recorded which did not have the classification of the case marked. 3) Calendar year 2016 had one case classified as Days Away from Work, but did not have the number of days away from work in Column L.
