On or about 7 June 2017, and at times prior thereto, various hand-held dispensing bottles within the shop floor contained flammable liquids that were not labeled to the contents or hazard of the chemicals, exposing the employees using them to a fire hazard.

On or about 7 June 2017, the employer did not complete the OSHA 300 Log of Work-Related Injuries and Illnesses in the detail as required by the regulation. 1) For calendar years 2014, 2016 and 2017, Column F, which requires a description of the injury or illness, parts of body affected, and the object/substance that directly injured or made the person ill, was missing the specific injury for each case that was recorded. 2) Calendar year 2014 had one case recorded which did not have the classification of the case marked. 3) Calendar year 2016 had one case classified as Days Away from Work, but did not have the number of days away from work in Column L.
