29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use AND AS REFERENCED BY THE NOTE CONTAINED IN 29 CFR 1926.103:    Twin Oaks Apartments, Interior Rooms:  On or about June 8, 2017, and at times prior thereto, employees were exposed to respiratory hazards, while painting indoor apartment rooms without a respiratory program to address key issues such as, but not limited to, training, fit testing, and medical evaluation for employees required to use a respirator.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:  Twin Oaks Apartments, Center of Complex:  On or about June 8, 2017, and at times prior thereto, an employee was exposed to a fall hazard while operating an aerial lift without a personal fall arrest system securely attached to anchorage points.
