29 CFR 1910.132(a): Protective equipment was not maintained in a sanitary and reliable condition: (a) Mayo Aviation: On or about June 8, 2017, and at times prior, workers were exposed to contact hazards from chemicals (including but not limited to: methyl ethyl ketone, nitric acid, chromic acid solutions) while using protective equipment that showed signs of degradation such as cracking and loss of flexibility or elasticity. 29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: (a) Mayo Aviation: On or about June 8, 2017, and at times prior, workers were potentially exposed to health hazards from wearing elastomeric respirators (including but not limited to: aggravation of pre-existing conditions due to not being medically cleared prior to use, dermatitis or ingestion of chemical due to improper storage of respirator) due to a lack of a written respiratory program which establishes procedures and elements for respirator use. Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: (a) Mayo Aviation: On or about June 8, 2017, and at times prior, workers were exposed to hazards (including but not limited to; diisocyanates, acetone, methyl ethyl ketone, or chromates) due to transfer containers not being labeled in a manner that provided at least general information regarding the hazards of the chemicals contained within.
