Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 6/07/2017, an employee of Antonio Cruz was removing roofing materials from a roof of three-story apartment building, building #10, located on Applerock Drive in O'Fallon, MO.  He was not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   The employee was observed on the roof of the building approximately twenty-seven feet above the ground without fall protection, exposing him to a fall to the ground below.
